Neuroscience Research Pre-Approval Form for a Non-OSU PI

Students seeking to enroll in Neurosc 4998 for research credit with a Non-OSU PI must first meet with a Neuroscience Academic Advisor. Once permission have been granted to use this form, you will need to submit this application by no later than the Friday BEFORE the first week of the semester in which their research experience will take place (retroactive credit cannot be granted).

Students are required to obtain input from their research site for the "RESEARCH SITE INFORMATION" and "RESEARCH MEMORANDUM OF UNDERSTANDING" sections; therefore, they should allot sufficient time to complete the form. Students will be enrolled in Neurosc 4998 as soon as their research applications have been approved. Such research experiences must be unpaid.

1) To use this form you will need to first meet with your Neuroscience Academic Advisor to obtain permissions to complete this from.

2) Keep in mind that any submissions without permission from a Neuroscience Academic Advisor will not be responded to.

3) This application MUST be submitted no later than the Friday BEFORE the first week of the semester in which their research experience will take place--No exceptions can be made.

4) You will receive an Approval or Non-Approval email within 5 working days of your submission.
